UIL ATHLETICS PREPARTICIPATION FORMS
Dear Parents and Student-Athletes:
According to UIL guidelines a pre-participation medical evaluation (physical) should be required upon a student-athlete’s entrance into Bowie’s athletics program.
In addition to following these guidelines, Bowie High School Athletic Training are committed to providing the most appropriate healthcare to each student-athlete, therefore the following paperwork must be completed and turned in to the Athletic Trainer BEFORE entrance into UIL athletics program:
- Pre-participation Physical Evaluation – Medical History Form
- UIL General Eligibility Rules - Parent/Student Information (comes with the physical form provided)
- Parent and Student Agreement/Acknowledgement Form – Anabolic Steroid Use and Random Steroid Testing
- Emergency Student Information Card
Important Things to Consider:
- UIL requires that each student-athlete completes the paperwork (3 forms provided)
- All paper work needs to be signed and completed before the student athlete can engage in any form of practice or workout. This means student athletes in an Athletics class will also need the paper work.
- Paperwork will be turned in to the Athletic Trainer ONLY. The student needs to have all 3 forms signed and completed before he/she can get released to participate.
- Athletic Trainers will be accepting paperwork for the 2008-2009 school year starting April 15 - June 2. We will then start picking up paper work at a later date before Football, Volleyball, and Cross Country start their "Two a days."
